Who issues your license

The Arizona Medical Board licenses MDs at azmd.gov, and the Arizona Board of Osteopathic Examiners licenses DOs at azdo.gov. Choosing the right board before you pay any fee is the single most important first step.

Eligibility requirements

You'll need a degree from an accredited medical school and at least one year of post-graduate training for US graduates. USMLE or COMLEX steps one, two, and three must all be completed within a seven-year window. You'll also need a notarized copy of your birth certificate or passport and a fingerprint-based background check.

How to apply

Applications go to the Arizona Medical Board for MDs or the Osteopathic Board for DOs. One thing to know, Arizona accepts credit card payments delivered by mail only.

What it costs

Online payment is not accepted, so build in extra days for your check to arrive. The initial application fee is six hundred and seventy-five dollars, non-refundable. Biennial renewal is five hundred dollars. If you renew after your expiration date, a three hundred and fifty dollar late penalty is added on top. Expect sixty to ninety days from submission to issuance for a standard filing.
