Who issues your license
Maryland licenses both MDs and DOs through the Maryland Board of Physicians. Two things make Maryland distinctive. Renewal runs on an alphabetical cycle tied to your last name. A through L renew by September thirtieth of even years, M through Z by September thirtieth of odd years. And Maryland requires a separate state-level controlled dangerous substances registration on top of your federal DEA.
Eligibility requirements
You'll need a degree from an accredited medical school, twelve months of post-graduate training for US graduates or twenty-four months for international medical graduates. USMLE or COMLEX with a four-attempt limit per step.
How to apply
Applications go through the FSMB uniform application. FCVS is accepted and recommended. Maryland CDS registration and PDMP enrollment are required separately from your federal DEA. Applications go through the FSMB uniform application. CHRC fingerprints must be submitted through a Maryland-designated vendor. They expire if not used within the Maryland-defined window.
What it costs
There's also a twenty dollar monthly fee that accrues from the date of approval through to your first renewal cycle. It's a small amount, but it surprises a lot of applicants. Initial application fee is seven hundred and ninety dollars for US graduates and eight hundred and ninety for international medical graduates, plus a twenty dollar prorated monthly fee from the date of approval through your first renewal. Biennial renewal is approximately four hundred and eighty-six dollars. The Maryland CDS registration is roughly one hundred and twenty dollars on a three-year cycle.
How long it takes
The board targets twelve weeks on average, though clean files can issue in under twenty days. The statutory review window is four months. Apply at least four months in advance. The IMLC pathway typically runs four to six weeks. The delays we see most often, the alphabetical renewal cycle catching physicians off guard. If your last name is N, your first renewal might arrive at a completely unexpected time. CHRC fingerprints expiring before they're matched to your application, and the Maryland CDS registration being overlooked when setting up prescribing authority.
